Athletics Forms

  • Parent Contact Form- Fill out online.  Very simple form so that coaches may contact you.
  • Sports Physical- to be completed by a qualified healthcare provider.  Sports physicals can be easily and affordably obtained through your family doctor’s office, CVS Minute Clinic, Duke City Urgent Care, NextCare Urgent Care etc.  Only one physical is needed per school year regardless of how many sports your child plays for APS.  
  • Concussion Course for Students- please have your player complete the following online course and print their certificate of completion.  On the webpage, select “New Mexico” and then “order course”.  There is no charge for this course.
  • APS Athletics Code of Conduct-  Please print, complete and turn in with your other paperwork.
  • APS Athletic Participation Consent Form-   Please print, complete and turn in with your other paperwork.
  • APS Parent Code of Expectations- There is no need to sign and submit this, but please take a moment to familiarize yourself and family members with these policies.  Taylor Athletics takes sportsmanship very seriously from both players, staff and parents/fans.
